I get to the point where I put in the license code, data base name, user name and password and everything shows OK except the Database User Name and password (the same I used to create the forum database.
What do I do now?

Eveything on that configuration page shows up with a green "OK" except for Database Username and password, which have a red "Failed"
I can confirm its not in my php or mysql configuration because I've installed another forum package as a test into a different database without any error.

If those are failing, then the database username and password could be incorrect. Check your web hosting control panel, or your web host for the proper database username and password.

No, I have confirmed that they are correct. They work fine when installing other php systems that create their own tables.
I have set permissions on the fusionbb files and folders as instructed.
I just deleted the empty database and created a new one with a different name (using the same username and password), and when I run install on the Fusionbb, still the same.

From memory, & I can't confirm until I redo my test install of 3.2 later this week, do you need to put your cpanel or equivalent username_ in front of the database name? eg username_dbname rather than just dbname.
I think I got caught out with this in the past.

SB, yes, that is correct, it must be "username_dbname. Same for the db user. For instance, on one of my sites the db username is "gregk_gregk" The db would then be, gregk_dbname.

Sorry, but that still fails in the same fashion.
User Name and password still show "Failed" on the page.

You're on a Cpanel server?
When you create the database, you actually have to go into your Cpanel and assign the database user to that database and give them "all Privileges". The usernames/passwords are database specific in Cpanel.
